FACILITY USAGE REQUEST FORM - rest Church Paducah Campus

In order to rent the facility, rest Church will collect the following information for approval before entering into a rental agreement. Please read the FACILITY USE AGREEMENT/TERM & CONDITIONS below and fill out the form in its entirety. Upon review, you will be contacted by a Deacon from rest Church. Decisions concerning applications for use of this building, their appropriateness, and the fees, are made by your designated Deacon or Pastor. rest Church reserves the right to refuse rental of the building to organizations and persons who are not in agreement with our core beliefs and values.

FACILITY USE AGREEMENT/TERM & CONDITIONS:

1. Provide a $200 deposit to the Deacon who contacted you via check. This deposit will be voided after the check out of your event (for Family Partners) - per everything is left better than you found it; non-refundable for non-Family Partners. Checks for deposit may be written to "rest Church," or if you want to make a donation you may Venmo your deposit. Any damages to the facility are under the responsibility of the renting party. At the conclusion of the event, the rental party must sign a completed clean-up check list. The deposit for Family Partners will be refunded after inspection of the facility. A credit card must be left on file at agreement of rental for all parties. 2. Please check with your Deacon prior to the event to discuss what you will need to do to prepare for your event. 3. The event will begin and end within the time established on this agreement. 4. SMOKING or VAPING is strictly prohibited within the rest Church building. 5. Consuming alcoholic beverages in any form on rest Church property is not allowed. 6. Those groups which expect to have children at their event: Children must be supervised by parents or caregivers at all times and stay within the rented area. If children go beyond rented area you will be charged rental cost for the room they enter. 7. Do not allow any person/child on the stage in the sanctuary at any time for any reason. If any damage is done to any equipment on or around the stage you will be responsible for replacing said equipment at cost; this includes the Jumbotron. 8. Use of any of rest Church's AVL (audio, video, lighting) equipment MUST be approved by your Deacon and will be operated by a rest Church Tech Team Member at an additional, non-refundable expense. Your Deacon will connect you with a Tech Team Member to make those arrangements. 9. The renter is responsible for set-up and break-down of all tables and chairs used at functions; tables and chairs will be returned to their original location. All furniture must remain in their respective rooms, unless prior arrangements have been made. If temporarily re-arranged, any furniture must be returned to the original placement. 10. As part of the cleanup after an event, the rental group will pick up all trash on the floors, tables, chairs and any other furniture that has been moved. The floor will be broom swept if necessary (broom and dustpan can be found in the kitchen). Any spills will be cleaned up. Pour any liquids out of cups before disposing of them. If needed, large black garbage bags are available in the kitchen for use. Garbage bags should be taken to the dumpster behind the building. New trash bags should be placed in the trash cans. 11. The kitchen is to be left clean – all garbage and recyclables are to be removed from the building immediately after any function to the dumpster. 12. All decorations, staging, and equipment must be broken down and removed immediately following the event. 13. Any personal or group property left on the church premises shall be at your own risk and only with prior permission from your Deacon contact. 14. No Free Roam of Children or Guests: Everyone attending your event must remain in the room(s) designated in your rental agreement. Do not access other areas of the building. Children must be supervised at all times and are not permitted to wander. Failure to comply will result in loss of your deposit and may result in revoked access to future facility rentals.

RENTAL FEES:

1. All groups using the building can reserve one or more of the rooms on the form below. You may expect to find others using portions of the building simultaneously. The fees listed are for one-time use of the facility. Other fees for on-going use may be negotiated with the Deacon team.

2. Scheduled building usage may include permission for light use of the kitchen. Each group must provide its own supplies. A kitchen tour will be given by your designated Deacon prior to your event if you request this. Full and exclusive use of the kitchen includes use of all appliances, dishes, cookware, etc., according to the instructions given by your Deacon. It will also provide assurance that you will not have to share the kitchen with another group. The kitchen must be left clean and neat with all garbage removed to the outside dumpster behind the building.

Rental Deposit: $200 ~refunded upon satisfactory completion of contract
*To rent the facility a credit card must be left on file (after form submission)
*Fees below are in association to non-Family Partners of rest (excluding particular organizations such as the Police Department)
*Regular Attenders of rest will receive a discount to fees below, and can be discussed after form submission

-General Rental Fee: $200 non-refundable
-Sanctuary Fee: $200 non-refundable
-Any other room Usage Fee: $100 per room ($50 per room for multiple rooms) non-refundable
-Stage Fee: $300 non-refundable - If anyone is found on the stage at any time (without prior permission) whether child or adult, for any reason, you will be automatically be charged $300. Any equipment that is damaged you will be responsible for replacing at cost.
-Technician Fee: TBD prior to event if applicable to your event; prices will vary.
-Time Fee: $40 per hour: The rental group will be charged overtime at a rate of $20 for every 30 minutes you exceed your allotted time slot.

I /we (the renters) understand and agree to the policies and rules set forth by rest Church as established on this document. I/we agree to pay a $200 deposit for the use of the facility. For Family Partners the deposit will be shredded upon successful checkout by a Deacon of rest Church. I/we accept the responsibility for any additional custodial fee or staff needed to host my event (minimum of $50) if the facilities used are not left in accordance with the policies of this document. It is my/our (renters) responsibility to pick up a building key and to arrange for any outstanding fee payment at least one week prior to the date of use. I/we agree to submit a credit card to be stored on file. I/we hereby release rest Church from any and all liability, past, present and future, arising from use of the Church facilities. Damage to the church property caused by the renter, or any person attending the event for which the church was rented, will be charged to the renter. Any infraction of this agreement may result in loss of security deposit, denial of further use of the church premises, and/or cancellation of this contract.
